A diagnosis of Dementia or Alzheimers is life changing for both the individual with the disease and their family and loved ones. Providing care for someone living with this disease is not a simple task and can place a great deal of stress on the caregiver. In the early stages of dementia, the individual may feel everything is under control but as the disease progresses so does the degree of care sometimes even leading to 24/7 assistance.
Our Care Managers will work with you and your loved one to develop a personalized home care plan and coordinate home requirements with hospital discharge planners, seniors residence staff and long-term care staff. And, with your permission, we’ll keep your loved one’s physician, seniors residence home or long term care facility up to date on how they are doing.
Services to support continued care at home
There are many illnesses, diseases, injuries, and situations where a person requires care that can be considered as “specialized”. The reason being that these situations require some particular considerations to be taken into account when providing care to such patients. At Offshore, we recognize these situations and the special considerations that are unique to these cases. We realize that the caregivers in such conditions need to have special training, a specific personality and/or a specialized expertise. Nutrition plays a significant role in many major health issues such as high cholesterol and heart disease, diabetes, as well as food allergies and intolerance; to name a few.
ParaMed’s Registered Dietitians provide individualized nutrition assessments and work with clients toward development of a treatment plan, including diet programs, meal ideas and recommendations on nutritional supplements, as well as nutrition counselling.
Our highly qualified dietitians will evaluate your current nutritional habits and develop personalized strategies for you to enjoy a healthier lifestyle. While some clients wish to establish a more nutritious diet, others are seeking a way to address and manage a particular health condition. In order to maximize long-term benefits, the focus is on setting achievable goals that will foster sustainable habits.
After a hospitalization, you will need help doing things that you’re unable to do for yourself — even with performing basic tasks like cleaning and dressing. You may need a nurse to change the bandage on a wound or to administer intravenous drugs. You may need equipment, too: a walker, a bath seat or a commode to ensure you don’t injure yourself during recovery.
Diabetes, arthritis, hypertension, lung disease. Chronic diseases like these can force seniors to give up their independence too soon. Offshore has proven programs that empower individuals to manage their own care and improve their quality of life. Home intravenous treatment is a way for you or your loved one to receive IV medicine without being in the hospital or going to a clinic. Intravenous care services provided by Offshore make it possible for clients to receive medications such as antibiotics or chemotherapy in the comfort of their home. You may need high doses of antibiotics or antibiotics that you cannot take by mouth. You may have started IV antibiotics in the hospital that you need to keep getting for a while after you leave the hospital. For example, infections in the lungs, bones, brain, or other parts of the body may be treated this way.
Offshore nurses provide comprehensive wound care management ranging from chronic to post-surgical. Our nurses are experienced in wound and ostomy care, and can help answer questions for the client or family caregiver. We focus on providing quality care for complex or hard-to-heal wounds. Our specialized wound care nurses are recognized for their expertise and highly effective, evidence-based treatments. We visit the home to assess patient needs and then work with the referring physician to develop a specific plan of care.
We know staying healthy and active is a priority for seniors — and for most, it is the key to successful aging. ParaMed nurses conduct in‑home regular check-ins to ensure you or your loves ones are staying hydrated, are eating well and are taking medications are welcome nursing visits that can reduce anxiety or stress. At Offshore, our nurses will also check vitals and work with you and your family to develop a customized Care Plan that can include wound care, disease management, intravenous care, client advocacy, medication management, chemotherapy and more.
Respite care services are essential to strengthening the ability of families and primary care givers to continue to provide extraordinary care in their home.
